    During deployment, when do you
    a) reveal your lieutenant (in Decapitation)
    b) designate your fireteam and team leader, i.e. can you keep one member of the fireteam as your reserve model and define the fireteam after he is deployed?

    a) When you deploy a model, you disclose all his open info. In Decapitation this includes the Lt status.
    b) only if the last model is the Team Leader.

You cannot add a member to the Fireteam as Reserve model.

It doesn't say you need to place the Team Leder Marker at the same time as you deploy a particular model. It just states that the team leader marker should be placed during the deployment phase. I could interpret that as whenever I choose, and hence wait with the marker till after the reserve model is placed.

as i see you get it all wrong:
you can place last model & make it team leader. no problem
you can place last model & say it is join existed fireteam (which were created in a "main" deployment phase). but you cant check Coherency for this last model before game starts.
you CANT place last model & in that moment create fireteam leading by another model.

this is how i see it from a link you provide.
it would be good if @ijw again confirm or explain this moment.

It is impossible to join existing fireteams without cancelling that fireteam and recreating it. The only exception is units that were previously joined and still in Fireteam Coherency during next order count.

@Mahtamori - it just what i see in a discussion here
to be honest - i newer reserve a fireteam member myself to avoid such questions, but i also newer mind my opponent create a fireteam as he want, even with reserved model...

https://forum.corvusbelli.com/threads/its-x-start-of-game-sequence.25748/#post-174326 is the most recent discussion.

Apart from the single exception that Mahtamori listed, there is no such thing as 'joining an existing Fireteam', only creating a Fireteam.

Why? Placing the Team Leader marker is not linked to the deployment of a specific trooper (unlike things like placing SymbioMates).
