JSA have Special Fireteam: Core Domaru, Kempei, 3 Keisotsu (not up to 3) Does this mean if any member of the fireteam is knocked out it is broken?
Not quite. Those restrictions are to form the Fireteam, not to maintain it It means that if the Fireteam is broken and you don't have exactly 3 Keisotsu available, you can't reform it using the Domaru. Keisotsu has another special Fireteam that allows 1 or 2 Kempeitai and up to 4 Keisotsu to form a Core. Example: Your Keisotsu HMG who is the leader dies. You can not reform 2x Keisotsu FO + Domaru LT + Kempeitai MSV2 You can reform using 2x Keisotsu FO + Kempeitai MSV2 Example 2: Your Keisotsu HMG who is not the leader dies. Domaru who is leader now has bonuses from a 4-man Core to play with. Team is not broken, team does not need reforming. Example 3: Both of your Keisotsu FO goes down. One Keisotsu FO is brought back by a Tokusetsu, the other is killed by the Tokusetsu. At the start of your next active turn, the Fireteam is still active and looks like this: Keisotsu HMG + Domaru LT + Kempeitai MSV2. Because the Keisotsu FO used to be part of the Fireteam and because the Keisotsu is in range of the Domaru leader, the Keisotsu FO will automatically be able to re-join the Fireteam even if it doesn't conform to the requirements in the JSA sectorial list - you are not reforming the Fireteam. End result is: Keisotsu HMG + Keisotsu FO + Domaru LT + Kempeitai MSV2
No not that it's broken. It's only broken in the usual way. Leader is taken out or something else. What it means is, that if the team is broken, it can't reform, unless it has those exact troops around. So if you have extra Keisotsu, they can replace one that dies. However you can re-form the other special team because it's Kempei+ up to X Keisotsu.

Ah, that one, it refers to the minimum number for the specific type of fireteam; e.g. Core, Duo, Haris, etc. and a special fireteam: Core is not a type in and of itself It's a very poorly written line since all Fireteams require 2 or more members. It has potentially been future-proofed, but nothing has ever come from that future-proofing. Tohaa Triads, for example, are still fine to form as two-person Fireteams.
Tohaa triads that get reduced to two members are fine. You still need three to form one (at the start of the game or later). All of the fireteam types should say something like this in Cancellation: In addition to the Cancellation conditions specified on the Infinity Fireteams rule, a Fireteam:X is cancelled when it is reduced to less than two members. Yeah, even Duo has that wording. But, like you said, they could write a fireteam that was different some day.
